Before you upload
- Include your reference ID exactly as issued
- Use your full name and current email address
- Upload clear, readable files only
- Use this form only for application-related records
- Keep a copy of what you submit for your records

- Clear JPG or PNG photos
- Utility bills or shutoff notices
- Housing-related documents
- Other requested supporting records
Tips before submitting
- Make sure names, dates, and balances are readable
- Show all four corners of the document when possible
- Use the same name and email from your application if possible
- Keep your reference ID for future status checks
